North East Hampshire and Farnham CCG intend to direct award to 4 existing integrated primary care providers operating within the geography of the Clinical Commissioning Group for Improved Access To Primary Care: — Farnham Integrated Care Services Limited (referred to as FICS within this document), — Oakley Health Group (referred to as Oakley Health within this document), — North Hampshire Urgent Care (referred to as NHUC within this document), — Salus Medical Services Limited (referred to as Salus within this document). The reasons for the proposal of this direct award are: — to provide consistency in delivery of a number of key locality based enhanced primary care services — recognising the positive impacts on patient outcomes and primary care provision that have been realised to date, — to recognise the contribution made by primary care super practices and federations in relation to the development of locality-based integrated care models throughout and now beyond the period of the “Happy, Healthy at Home” Vanguard Programme, — to recognise the vital importance of local clinical leadership in the development, mobilisation and success of enhanced primary care services, — and to support the continued essential role of enhanced primary care services within the out-of-hospital environment. The initial contract term will be 3 years with a possibility of a 2-year extension comprising of 1 year and 1 year. The period of time covered by this direct award is from 1.4.2019 to 31.3.2022 (to 2024 with the 2 extensions of 1 year each).
