The service is currently using a solution which is due to be decommissioned. . The component allows us to create and manage rules for selecting participants who need to be invited to be invited for breast screening in Wales The existing system is called the National Health Applications and Infrastructure Services (NHAIS) which is run by NHS England and is due to be switched off and decommissioned in which estimated dates for Wales for March 2024. We are undertaking a further discovery, specific to NHAIS to identify which of our user needs are currently provided by NHAIS and which will make up a minimum viable product to replace this function This tender includes both Alpha and Beta development of a replacement component which can be used to replace the cohort functionality of NHAIS for breast screening cohort selection. The component should be designed with the intention of using it in other screening services. If the minimum viable product in the discovery includes more functions than the cohort tool then we will need to investigate the most suitable home for the extra functions. We would like a partner to work alongside our developers and business owners, helping us to learn new ways of working and pairing while we deliver. Tasks may include testing the rules set we will need to apply to our demographics database, considering what, if any, containerisation can be done to the existing breast test system to make it easier to apply a new set of selection rules. Depending on the outcome of the Alpha, the minimum viable product may be close to completion or not yet close. Development of a secure, viable product with time for integration and QA testing would be essential within the strict timescales of March 2024. During Beta, tasks could include working closely with our data engineers and developers to prepare for deployment and maintenance of the component, user acceptance testing, training and formal move from NHAIS. Diabetic Eye Screening Wales: We have recently completed an alpha with diabetic eye screening and are developing our use of Optomize to improve our service. One part of our alpha included a successful proof of concept to automate the reading of new referrals to our service. We would like a partner to work with us to help us turn the proof of concept into a functioning component for our diabetic eye screening service. Where the supplied staff will work Wales Where the supplied staff will work No specific location (for example they can work remotely) Why the work is being done The component that selects people for invitation to breast screening in Wales is called National Health Application and Infrastructure Service (NHAIS) and is currently delivered by NHS England and Digital Health and Care Wales. But NHAIS is end of life and is due to be turned off and decommissioned for Breast Test Wales in March 2024. We need to continue to work with partners in both NHS England and NHS Wales to replace it before then. Due to the different rules that are applied in Wales, the tool will need to be different from the solution in England, though it will still need to exchange information. Ideally the tool will be flexible enough to select cohorts for other screening services in the future. Diabetic Eye Screening Wales is performing manual entry activities that could be automated, leading to more time for specialists to manage diabetic eye screening activities. The business problem you need to solve Breast cancer screening The aim of the breast screening programme is to reduce mortality from breast cancer. Women aged 50 to 70 who are resident in Wales, and registered with a General Practitioner, are invited for a mammogram (X-ray of the breasts) every three years. Breast Test Wales is divided into three geographical divisions with centres in Cardiff, Swansea, Llandudno and Wrexham. Eleven mobile units work across Wales to provide local screening to women who live some distance from a centre, visiting over 100 sites in every three-year round of screening. People who should be invited to breast cancer screening need to be identified before an invitation can be sent. The component that selects people for this invitation in Wales is called National Health Application and Infrastructure Service (NHAIS) and is currently delivered by NHS England and Digital Health and Care Wales. But NHAIS is at the end of its life and is due to be decommissioned and this will be from March 2024 for Breast Test Wales. We need to replace it before then, ideally with a tool flexible enough to select cohorts for other screening programmes in the future. Diabetic Eye Screening Wales The service aims to detect diabetic retinopathy before sight loss occurs. Research evidence shows that with early identification and treatment, loss of vision can be prevented in 70 – 90% of people with sight threatening diabetic retinopathy1 2 . People aged 12 and over with a diagnosis of diabetes, who are registered with a GP in Wales, are eligible and are invited for regular retinal screening with DESW. Around 15,000 people a year receive new diabetic diagnoses that mean they are eligible for eye screening as part of their ongoing health care. Currently, the referrals come in by physical mail or email and must be uploaded manually. We wish to develop our proof of concept automatic reader into a live component. In this way we hope to release around 4,000 hours of our specialists’ time from manual entry tasks, so that instead they can help more people to be screened. Expected deliverables Working live deployments for both breast test cohorting and diabetic eye screening referrals tools. The work should be completed through mixed teams, with partners and Public Health Wales teams working alongside each other to deliver. The work and deliverables should meet the Welsh digital service standards, and we should be meeting the principles laid out in the Public Health Wales digital and data strategy. Although we do not specifically require individuals to be security checked, we expect all members of our partner team to have undergone appropriate checks and training within their organisation. Our partners must hold either ISO27001 or Cyber Essentials Plus certification. They will also need to apply for a code of connection to NHS Wales and work closely with us to ensure the safety and protection of sensitive information throughout our partnership.
