Opportunity to Provide Community Dermatology Services covering the Mid Hampshire area of NHS Hampshire, Southampton and Isle of Wight CCG (HSI).

Hampshire, Southampton and Isle of Wight CCG are publishing this call for competition, for community dermatology services for the Mid Hampshire Locality only for a period of two years. A two year contract is required in the Mid Hampshire locality to allow the HSIOW CCG to undertake a review of the community dermatology model and services from August 2023 in order to cover an interim period due to the following upcoming changes: The Mid Hampshire locality currently has a Community Dermatology service model, which was commissioned by the previous Clinical Commissioning Group (CCG). In April 2021, the North & Mid Hampshire CCG merged with five other local CCGs to become part of a single Hampshire, Southampton and Isle of Wight CCG (HSIOW CCG). In July 2022, the HSIOW CCG will in turn be dissolved with its functions transferring into a new HSIOW Integrated Care System (ICS). The integration of CCGs and establishment of the ICS provides an opportunity to establish a service improvement approach across a wider geography, in order to deliver improved outcomes, experience and efficiency. As such, this has the potential to change the landscape and alter requirements, as does the lessons being learned from the Covid-19 pandemic, which has resulted in different ways of working and patient/clinical behaviours. In respect of the Dermatology service specifically, our intention is to undertake a thorough review of the current service model and provision, encompassing service redesign and reconfiguration options, and population insights, as well as looking to the market for innovations in Dermatology services. This review will inform longer term commissioning strategies and potential tender opportunities for Community Dermatology. The Authority intends to award a two-year contract for Community Dermatology in North and Mid Hampshire The Service will; • Deliver a comprehensive Consultant Led Tier 2 Community Dermatology Service which provides appropriate and timely diagnosis and treatment of patients in a community setting • Provide a higher level of knowledge, experience and skill than usually found or expected in Primary Care therefore not duplicating services provided by an element of the core GMS/PMS contracted activity of through Direct Enhanced Services for Minor Surgery Agreements. • The service will consist of a staffing model consisting of but not exhaustive; Consultant knowledge, skills and clinical supervision, GPwSI skills, Nurse Specialists and general nursing support • Relieve pressure on acute Dermatology services at Hampshire Hospitals NHS Foundation Trust, particularly for suspected cancer referrals by forming close working relationships and pathways of care with the local Trust, ensuring only those patients requiring secondary care interventions are referred on • Continually improve quality and patient experience with reduced waiting times and delivery of one stop assessment, diagnosis and treatment services for all patients where appropriate. The catchment area covers 4x core Primary Care Networks consisting of 18 GP practices and a population (raw list size) of 228,041 people. There are also an additional 2x eligible PCNs consisting of 6 GP practices sat within the South West catchment area that due to their geographical location and patient choice, may refer a percentage of their patients into this service (raw list size 65,796).The new 24 months contract will be subject to TUPE of existing staff from the incumbent provider. This notice is an opportunity to express any interest in the delivery of the interim contract however if no expressions of interest are received from alternative providers, the CCG intends to extend the e contract to its incumbent providers to ensure service continuity whilst the service review is undertaken, under the Regulation 32 exemption from procurement if no expressions of interest are received. Lot 1: The Community Dermatology Service will provide high quality appropriate and prompt clinical care for a wide range of dermatology conditions in a community based setting. The commissioned service will be expected to offer initial assessment, diagnosis and relevant treatment together with appropriate follow up care for dermatology patients being treated by the service, in line with NICE guidelines, for example the treatment of low-risk BCCs. Service provision will be for patients aged 18 years and over who are registered with a Mid-Hampshire GP. The provider will be responsible for the supply, safe installation, on-going maintenance and replacement as necessary of all suitable and relevant equipment and software required to provide the service. The service provider will be responsible for ensuring there is appropriately qualified staff necessary to deliver a fully accredited service, compliant with all appropriate national standards and regulations, including business continuity. The provider will co-ordinate and manage the population need for Dermatology services, reducing dependence on referrals into secondary care; activity levels to be contained within agreed plans; covering provision of Advice and Guidance, Teledermatology, Face to Face and virtual modes for delivery of first and follow up appointments where clinically appropriate. They will develop strong local clinical relationships with both Primary and Secondary Care clinicians and work collaboratively to deliver and ensure patients receive an integrated, seamless pathway of care. The provider is responsible for the provision of the team(s) and administration support required to manage the service. The service must accommodate all dermatology patients with the exception of: • Patients under 18 years of age at time of referral • Suspected Cancer 2WW referral • Patients who require Phototherapy treatment • Suspected connective tissue disorders • Systemic illness with a rash • Cutaneous vasculitis • Immunosuppressed or have Gorlin's syndrome • BCC Lesions that: - Are on the nose and lips (including nasofacial sulci and nasolabial folds), or around the eyes (periorbital) or ears. - Are greater than 2 cm in diameter below the clavicle or greater 1 cm in diameter above the clavicle unless they are superficial BCCs that can be managed non- surgically. - Are morphoeic, infiltrative or basosquamous in appearance. - Have poorly defined margins or • Are located: - Over important underlying anatomical structures (for example, major vessels or nerves). - In an area where primary surgical closure may be difficult (for example, digits or front of shin). - In an area where excision may lead to poor cosmetic result. • If any of the above exclusion criteria apply, or there is any diagnostic doubt, the patient should be referred to the Local Service Multi-disciplinary Team. • The exceptions described above and high risk BCC's will be referred to the appropriate Secondary Care provider under the 2 week wait (urgent cancer referral guidelines). • Conditions which are excluded from NHS funded treatments (See Schedule G) Full detail available within the Service Specification which can be accessed via the in-tend portal under project ref F15204 as per additional information section. It is anticipated that the legal landscape governing procurement of NHS contracts is due to change in 2022. The CCG reserves the right to amend or abandon the procurement process in response to ay legislative changes that may occur. The Quality Evaluation model will cover areas such as Service delivery model, workforce, clinical and governance quality, safeguarding, IG, Equality and Diversity, mobilisation planning and assurance for service delivery as well as detail on TUPE process and transfer of staff which will be applicable to this service.
