This is a call for expressions of interest to attend a Market Engagement Event for NHS Staffordshire and Stoke on Trent ICB’s Homeless Health Service. The aim of the session is to gauge potential interest, inform the market of the Commissioner’s plan for this service and any potential future procurement, and review the potential procurement process. If a procurement is pursued by the ICB, it will be open to all suppliers regardless of whether they attended the Market Engagement event. The Market Engagement event will take place on Tuesday 11th October from 10.00am to 12.00pm at Castle House, Barracks Road, ST5 1BL. For technical support in accessing the PQQ please contact the BravoSolution Help-desk. Lot 1: The combined registered population of North Staffordshire and Stoke-on-Trent is 479,373. It is estimated that there are 4,975 people in Stoke-on-Trent experiencing combinations of homelessness, offending, and substance misuse. 1,090 people are estimated to experience homelessness in Stoke-on-Trent and 1,280 in Staffordshire. At any one time in Northern Staffordshire, there are more than 200 people in homeless hostel or night shelter type services. The Local Authority in Stoke-on-Trent has recently quoted a throughput of 900 people in these services. At any one time in Newcastle-under-Lyme there are between 12 – 16 individuals in temporary accommodation and up to 10 rough sleeping. The Staffordshire ICB is looking to commission a service which will co-ordinate care for vulnerable people who are homeless, including rough sleepers, within the Newcastle-under-Lyme and Stoke-on-Trent geographical area. The service is expected to be nurse led and must demonstrate a collaborative and whole system approach to prevention, assessment and provision of health interventions, that improve service users’ overall health outcomes and recovery. The Service will require extensive multi-agency working, it will particularly need to work in partnership with Stoke-on-Trent City Council and Newcastle-under-Lyme Borough Council. The service will need to make links with a wide network of services; working collaboratively across a range of multi-disciplinary teams and VCSE organisations to support the needs of these vulnerable individuals. This may include but is not limited to: • Voluntary Sector Services • Local Authorities – Housing services, social care • Other accommodation and support providers e.g. Concrete, Brighter • Futures, Lyme Trust • Substance misuse services • Primary & Intermediate Care – GPs, practice nurses, • NHS Hospital Trusts – Royal Stoke, Harplands • Mental Health services • West Midlands Ambulance Service Key objectives of the proposed service: • Provide education and raise awareness for local health and social care economy professionals, particularly but not limited to, A&E staff, ward based nurses, district nursing, community mental health, substance misuse services, ambulance services, GP reception staff, • Provide remote access to healthcare via a community outreach vehicle (COV). The COV should provide a clean, safe and confidential environment where clinical assessment and treatment can occur. Staff should have access to Wi-Fi and remote access to GP and hospital clinical records. Real time information should be shared with the patients GP practice about contacts and interventions the Homeless Healthcare Team have offered • Provide treatment and advice, including first contact see & treat to those sleeping rough or in hostel / shelter accommodation • Deliver basic primary healthcare, coordination, and prescribing to improve the person’s health and wellbeing in the community, in accordance with relevant plans; e.g. hospital discharge, etc. • Provide verbal and written information about common health problems (and where to seek further advice) including but not limited to; skin infections, chest / UTI infections, sexual health, needle exchange, blood borne viruses, alcohol / drug addiction, mental health • Hospital Liaison – to support care / discharge planning for people who are homeless • Support the development of multiagency care plans for people who are homeless and have multiple comorbidities; including attendance and links to relevant multi-disciplinary meeting The estimated annual budget for this service is £200k. Should this service be subject to a procurement, it will be advertised separately, and providers will need to register again when the opportunity is advertised. Further information relating to the service specification, as well as total budget and contract duration, will be made available via ITT documentation - should the ICB commence procurement of the service.
