The Specialist Floating Support service will: • Support adults with multiple disadvantages stemming from a combination of needs that would include mental ill-health, substance misuse, forensic background and behaviours of distress and who as a result find themselves at risk of becoming homeless or who are homeless. • Provide personalised and coordinated approach to the support of individuals. • Support individuals to achieve their outcomes and promote their independence. • Work with existing community groups part or outside the Open Mental Health Alliance and system partners such as Housing, Health, Adult Social Care and Probation services. Lot 1: Interested organisations must be able to meet the following requirements: • Have a base in Somerset • Have an extensive experience in supporting people with multiple disadvantages including people with complex mental health needs, trauma, substance misuse and offending history. • Be skilled in supporting people who can be reluctant to engage with services • Support a trauma informed approach to practice. • Provide robust support to a staff team who will be working with people with complex and challenging situations and stories • Be willing to work in partnership with other community- based and statutory organisations to enhance the support offer to individuals • Be prepared to be part of developmental work and the need to adjust the model of support and align the service to the changing landscape of mental health and housing services?
