Items (i) and (ii) are made from these blanks, with no expected major change in dimensions outside of material loss from lapping/polishing (no need to cut or grind to dimensions). Dimensions of the blanks for faces size, thickness, diameter, etc. supersede the ones from the criteria below. The remaining criteria are desirable (D) refinements on the polishing quality as well as coating options - Please state what can be done and at what cost, with several options if relevant.
