HOUSING RELATED SUPPORT PATHWAYS

The Housing Related Support Pathways are being procured following a review to understand the needs, demands and supply of housing related support in Wakefield. The pathway services model was designed as a result of the review and included a single point of access (SPOA) role within the Council to manage referrals into the pathway services. Lot 1: Assessment Pathway Assessment Pathway Wakefield Council wishes to commission an Assessment Pathway, comprised of: • Preventing Rough Sleeping Service • Assessment Centre • Short term Supported Housing Key aims of the Pathway will be: • To meet the needs of rough sleepers and prevent rough sleeping in Wakefield • To create an opportunity to identify and understand a person's support need and identify the most appropriate support for them • To support people to overcome barriers to gaining a tenancy • To promote independent living skills and increase the service user's ability to manage and maintain a tenancy • To support people to address needs around their mental health, substance misuse and/or offending behaviour • To create opportunities for change and progression • To promote positive approaches to health and wellbeing Lot 2: Access to Housing Pathway Access to Housing Pathway Wakefield Council wishes to commission an Access to Housing Pathway which will provide housing related support to people who are at risk of homelessness and need support to both find and maintain suitable housing. The Pathway will support people to find and access accommodation and provide outcome-focussed housing support in order to enable them to gain the skills to live independently. Key aims of the Pathway will be: • To support people facing homelessness in the district to access a range of housing options • To promote the use of the private rented sector as a viable housing option. • To support and enable people to plan for and achieve the best, realistic, housing solution for them • To promote independent living skills in a way that maximises the service user's ability to manage and maintain their housing • To promote positive approaches to health and wellbeing • To promote opportunities for training, employment and volunteering • Use of the Access to Accommodation Fund as an incentive to landlords to accessing the private rented sector Lot 3: Housing Sustainment Pathway Housing Sustainment Pathway Wakefield Council wishes to commission a Housing Sustainment Pathway which will provide housing related support to people who are at risk of homelessness to enable them to keep and maintain their housing. Key aims of the Pathway will be: • To prevent homelessness • To promote independent living skills in a way that maximises the service user's ability to manage and maintain their housing. • To increase service users' confidence in and ability to respond to issue with their accommodation • To promote positive approaches to health and wellbeing • To promote opportunities for training, employment and volunteering The Pathway will work with 145 single people and couples and 45 families (a total of 190 households) at any one time. Lot 4: Young Families Service Young Families Service Wakefield Council wishes to commission housing related support service to work with young families. Key aims of the Service will be: • To improve the life chances of young parents and to enhance the relationships between the parents and their children • To promote young parents independent living skills and increase their ability to manage and maintain a tenancy • To create opportunities for change and progression • To promote positive approaches to health and wellbeing The Service will provide 4 units of accommodation for teenage mothers and a floating support service for 32 young families (including mothers and fathers). Lot 5: Supported Housing for People with Mental Health Needs Supported Housing for People with Mental Health Needs Wakefield Council wishes to commission a supported housing service for people with needs around their mental health. Key aims of the Service will be: • To promote recovery and support people to build confidence and resilience • To promote independent living skills and increase the service user's ability to manage and maintain a tenancy. • To address barriers to gaining a tenancy • To promote positive approaches to health and wellbeing • To promote opportunities for training, employment and volunteering. The service will support people living in 12 units of accommodation at Woodside and Woodview in Normanton. Lot 6: Complex Needs Service Complex Needs Service Wakefield Council wish to commission an accommodation-based service to provide housing support to people with complex needs. Key aims of the Service will be: • To promote independent living skills and increase the service user's ability to manage and maintain a tenancy • To focus on harm reduction, recovery and resilience • To support service users to address their drug and alcohol issues by working in partnership with appropriate specialist support services • To prevent re-offending • To create opportunities for change and progression • To promote positive approaches to health and wellbeing The service will support people living in 8 units of accommodation at Springhill Close in Outwood, Wakefield.
