Sheffield Children’s NHS Foundation Trust would like to implement a Link Worker service within 3 defined pathways to address health inequalities:<br/>- Regular Emergency Department (ED) attendances<br/>- Dental Extraction Pathway<br/>- Complex medical needs<br/><br/>We would like to engage with the market via this Prior Information Notice as a request for information to help shape our specification for a Link Worker service. <br/><br/>Please see Atamis reference C302479 for access to our draft documentation including our specification.
