Greater Huddersfield and North Kirklees CCGs are wanting to procure a single provider for the provision of domiciliary care services for adults aged 18 and over following assessment in line with the Fast Track Tool as described in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care November 2012(revised). The aim of the Continuing Healthcare Fast Track service provision is to ensure the delivery of a robust, high quality and sustainable service which meets the requirements of the National Framework (updated 2018), is in line with the local CHC Commissioning Principles Policy (2015) and provides individualised patient care to all service users across Kirklees.
