Community-Based Physical Activity Programmes for Chronic Disease (Tier 2).

The HSE hereby invites suitably qualified and experienced service providers to submit tenders for the provision of a community-based exercise referral programme for people living and or at risk of chronic disease. Service users will be referred from acute, sub-acute and community based settings in the Dublin and Midlands RHA. There is a wealth of evidence for the benefits of being physically active, and its association with chronic disease prevention and management. Despite this, the Irish population remains largely inactive and there is a need to intervene. The HSE are in the process of developing and implementing a Physical Activity Pathway in Healthcare PAPH which is a set of steps a healthcare professional can take to encourage people to be more physically active. These include: 1. Screening Raise the issue and assess levels of physical activity 2. Intervention Brief intervention and person-centred physical activity advice 3. Participation Sign-post/refer to supports that will help them to be more active 4. Review Check in on progress The tender will be in four lots as follows: Lot Location Lot 1 Dublin South City and West Lot 2 Dublin South West Lot 3 Kildare and West Wicklow Lot 4 Midlands Tenderers may tender for Lots 1, 2, 3 or 4, 2 Lots, 3 Lots or ALL Lots.
