South East London Integrated Care Board (SEL ICB) Lewisham Borough are seeking to procure 14 Intermediate Care Beds based within the London Borough of Lewisham. The main service outcome will be to deliver a 14 bed based Intermediate Care bedded rehabilitation and reablement unit in Lewisham, providing nursing and HCA support element to the service to people who are medically optimised at the point of discharge but still requiring nursing due to the acuity of their clinical presentation. The provider will operate 24 hours a day, 7 days a week and 365 days of the year and ensure there is adequate staff to support patients in the beds. The maximum length of stay in these rehab beds is six weeks, during which time the service aims to improve the patient's functionality and physical ability. Anticipated service outcomes include a measurable improvement in patient functionality and reducing the risk of an avoidable hospital admission. To assist the Commissioners with testing its assumptions and proposed service requirements, a market engagement event will take place on Friday 13th December 2024 to explore the market's feedback on the following areas: • outline of service requirements and KPI's • timescales for procurement and mobilisation of the new contract • proposed contract duration and financial model Intermediate Care Bed Market Engagement Event Information Date of event: Friday 13th December Time of event: 11.00am - 1.00pm (followed by a light lunch) Address: Rooms 304 & 305, 3rd Floor Laurence House, 1 Catford Road, London, SE6 4RU Number of attendees: Maximum of 2 people per organisation Please complete the Market Engagement Event Registration Form by no later than 12:00pm (Noon) on Monday 09 December 2024 if you are interested to attend the event: https://forms.office.com/e/xq7xVMXmaC Lot 1: Background Intermediate care is a short-term intervention to preserve the independence of people who might otherwise face unnecessarily prolonged hospital stays or inappropriate admission to hospital. Bed-based intermediate care aims to prevent unnecessary admissions to acute hospitals and premature admissions to long-term care, and to support timely discharge from hospital. For most people, interventions last up to 6 weeks. Outline The service will provide a 14 bedded unit with nursing input and will cover patients over the age of 18 who are registered with a Lewisham GP and reside in the London Borough of Lewisham. The service will have capacity to support 4 double-handed patients at any one time though the provider will be expected to give consideration to supporting a higher number of people with initial double-handed support needs at point of hospital discharge. There will also need to be at least 2 beds designed and allocated to meet the needs of Bariatric patients. The therapy element of this service will be delivered by a multidisciplinary team commonly led by healthcare professionals but including front line care staff (in care homes). Therapy input will be delivered by the Lewisham and Greenwich NHS Trust (LGT) Therapy Team with programmes supported by the care home / provider staff. The provider is requested to include nursing and care/support staff, but not professional therapists or social workers in their bid. Aims Bed-based intermediate care addresses those instances when a person's level of need means they require intermediate care in a setting other than their own home. In Lewisham bed-based intermediate care is a time limited episode of care (of up to 6 weeks), commissioned by the SEL ICB (Lewisham) and supported by the Local Authority. The Provider will make available adequate space and equipment to provide physiotherapy and occupational therapy to enable patients to pursue their rehabilitation goals. The provider will need to accommodate the Therapies Team within a secured office environment as well as ensuring they provide adequate staffing to patients in the 14 allocated beds. The 14 designated beds would require en-suite facilities. Meals and refreshments will be provided to patients accepted for bed-based rehabilitation. Patients are referred to bed-based intermediate care either as a 'step up' from home for assessment and rehabilitation - as an alternative to hospital admission - or as a 'step down' from the acute hospital for a period of assessment and/or rehabilitation before they return home. Referrals to the service will be accepted up until 7pm, 7 days a week and transfers will be arranged through the referring Trust. The main service aims of the service are; • Supporting with timely and accurate delivery of rehabilitation and reablement programmes. • to provide a person-centred holistic assessment and provide therapeutic outcome driven goals to increase independence. • promote faster recovery from illness. • prevent unnecessary hospital admissions and premature admissions to long term residential care. • reduced incidence of UTI and CAUTI. • maximised nutritional status. • Waterlow risk assessment to manage potential incidents of development of pressure ulcers and provide effective wound care management. • Improved health status such as gaining confidence by restoring & maximising independent functioning/living. • Safer staffing ratios to support the delivery of bed-based rehabilitation and supporting patients 24 hours a day, 7 days a week. The indicative maximum financial envelope for this procurement will be £989,000 per annum. SEL ICB however would expect to incentivise bids below this amount through a scoring methodology applied. The contract will have an initial term of 3 years, with the possibility of extending the term up to a further 2 years beyond the initial contracted duration. This will be subject to satisfactory performance and by mutual agreement between the Commissioners and the Provider.
