Blaby District Council wishes to invite tenders for the supply of services to support the residents of Leicester Leicestershire and Rutland who have experienced a period of illness and have found themselves in hospitals faced with a housing related barrier to discharge. Lot 1: • To support a safe discharge home following admission to hospital (quick turnaround to free up hospital beds) • To reduce the risks / hazards in a property and support residents to live independently at home and to have informal or formal care.
