Southampton City CCG intends to direct award a contract for the Acute Visiting Service (AVS) and Enhanced Health in Care Homes (EHCH) services to Southampton Primary Care Ltd (SPCL) Contract for 1 year, from 1 April 2020 until 31 March 2021. Due to the timeline for working with PCN CDs to identify the models moving forward, the CCG are intending to award directly to the incumbent provider, SPCL, as an interim solution. Southampton City CCG intends to direct award a contract for the Acute Visiting Service (AVS) and Enhanced Health in Care Homes (EHCH) services to Southampton Primary Care Ltd (SPCL) Contract for one year, from 1 April 2020 until 31 March 2021. The Acute Home Visiting Service provides acute home visits on behalf of all practices in Southampton but does not fully replace practice home visiting arrangements. The service enables timely home visits, early assessment, investigation and discharge to an appropriate health professional in the community, an integrated approach to home visiting, improved patient experience, smoothing of peaks in demand for urgent care services throughout the day, reduction in number of non-elective admissions or length of stay and reduction in health risks associated with hospitalisation. The Enhanced Health in Care Homes service complements the GMS core contract by providing EHCH locality teams have a regular presence and provide a ‘named clinician’ to each of the homes across the city and visits all new residents and all patients after an incident of concern (e.g. hospital admission, out of hours ambulance call and A&E attendance) to either undertake their initial Comprehensive Geriatric Assessment or review their existing assessment (including medication reviews) and support the development/update of their care plan.
