Lincolnshire Admission Avoidance and Discharge Service (LAADS) Prior Information Notice (PIN) This is a Prior Information Notice (PIN) undertaken as advance notice to the market that a future procurement opportunity is likely to be advertised in September 2021 with an aim to mobilise the successful provider implementing a new service by March 2022. The contract will commence 01 March 2022 for t3 years, with the option to extend for up to 24 months. The service contract will therefore cover a maximum duration of up to 5 years. The aim of the procurement activity is to source suitable, capable, quality provision of health care and support services for patients across Lincolnshire, a county in the East Midlands of England, in the United Kingdom of Great Britain. The service will be procured by United Lincolnshire Hospitals NHS Trust (ULHT) Lincolnshire Procurement Service on behalf of and for the lead commissioner and contracting authority: Lincolnshire Community Health Services (LCHS); working in collaboration with Lincolnshire County Council (LCC). Additional updated information will be provided at the point of advertising the opportunity including service specification, indicative activity data / demand forecasts, contract values, and any other relevant information required to respond to the opportunity adverts and call for competition at that time. The financial figure published on this prior information is a maximum prospective value taking account of broad service requirements for which the contracting authority is reviewing the funding position. The full service specification, contract terms and conditions, and procurement documentation including timeline will be published with the advertised opportunity, call for competition, and invitation to tender where providers will be able to submit their completed tender responses via the eTender portal: Proactis Due North. Providers can register in advance as suppliers on Due North here: https://procontract.due-north.com/Login where all communications regarding advertised contract opportunities will be managed by Lincolnshire Procurement Service on behalf of the named Contracting Authority. Lot 1: Lincolnshire Admission Avoidance and Discharge Service (LAADS) will be a health and social care service procurement seeking suitable provision of health and social care support services. It will respond to requirements of service users by providing packages of care in line with the service specification and service key performance indicators to avoid unnecessary hospital admissions and support hospital discharge of patients through domestic care and support provision. The provider will also be expected to facilitate care plans in the community and access to other services where individuals need this support. The service will deliver short term packages of care as support interventions to reduce hospital admissions. The service response team will provide timely and responsive support to people who have been assessed as needing short term support or further assessment within their own home, domestic environment or other discharge residence destination, in line with Home First and Hospital Discharge Guidance. The key requirement of this service is to support a reduction of admissions to hospital and provide support to allow people to remain at home during an episode of ill health or where current care provision becomes unavailable through the provision of individualised packages of care in their own home. The service may also be required to provide short-term packages of care to support those who are medically optimised to allow them to be discharged from hospital at the earliest point. Urgent Community Response provision is also being considered in relation to this service, wider health and social system service reviews and may be a consideration for service development. LAADS complements a number of commissioned services within the local health and social care economy as part of an integrated care system. LAADS will be utilised where an immediate need for a package of care (health, social or both) is required and an alternative service cannot provide the necessary support in the immediate short term time frame required. Providers may wish to research and consider current national Home First and Hospital Discharge Guidance in relation to this service in preparation to responding to the invitation to tender for this contract opportunity.
