The Council is conducting the procurement exercise under the Light Touch Regime (LTR) Design Procedure set out in chapter 3 section 7 of the Public Contracts Regulations 2015 (PCR 2015) and the Public Procurement (Amendments, Repeals and Revocations) Regulations 2016.There is a requirement for the provision for multiple disadvantaged vulnerable adults. This often leads to health problems and social exclusion being exacerbated and the potential to rely on crisis or statutory services and high-cost care later in life. Citizens in scope will be experiencing or have experienced two or more of the following: • substance misuse;• poor mental health;• a history of repeat homelessness; • a history of offending;• those who experience serial exclusion from services or are at risk of exclusion from mainstream services. Lot 1.1: Youth Hub (16yrs – 24yrs) Provision of guidance and information, support and connection to services, delaying or preventing the need for more costly interventions. Client Group - Young People (16-24yrs) Lot 1.2: Young Persons Lead Worker (18 -24 years) Provision of strength-based support to citizens in the community for a period of time, usually up to 2 years. Lot 1.3: Young Persons Lead Worker Plus (18 -24 years) Provision of strength-based support in long term accommodation (2 years) provided by the service provider. Lot 2.1: Community Based Advisors Quality advice, guidance and signposting across multiple access channels offering citizens choice of preferred contact method. Lot 2.2: Adults Lead Worker (25 years plus) Provision of strength-based support to citizens in the community for a period of time, usually up to 2 years. Lot 2.3: Adults Lead Worker Plus (25 years plus) Provision of strength-based support in long term accommodation (2 years) provided by the service provider. Lot 2.5: Discharge to Assess (Health and Homeless) Service (18 years plus) Provision of intensive strength-based support for people with complex needs who have no suitable accommodation to return to at the point of discharge from any bedded provision e.g. acute, residential. Lot 3.1: Domestic Abuse Front Door Hub (16 years plus) Provision of guidance and information, support and connection to services, delaying or preventing the need for more costly interventions. Lot 3.2: Domestic Abuse Lead Worker (18 years plus) Provision of strength-based support to citizens in the community for a period of time, usually up to 2 years. Lot 3.3: Domestic Abuse Refuge Spaces (18 years plus) Provision of emergency accommodation for a period of up to 12 months. Lot 3.4: Domestic Abuse Dispersed Accommodation (18 years plus) Provision of self-contained units and support for up to 12 months. Lot 4.1: Lead Worker Plus (18 years plus) Provision of strength-based support in long term accommodation (2 years) provided by the service provider.
