Award under Direct Award process B (there is therefore a mix of new and existing providers) All must fulfil the following criteria: The provider meets all requirements in relation to the provision of the health care services to patients • Provider required to produce evidence of relevant registrations / qualifications for all clinicians delivering the service: Name: Registration/licence to practice: Date of FSRH letter of competence in LOC IUT Date of FSRH letter of competence in LOC SDI Date of FSRH Faculty Registered Trainer Status Date of recertification* For doctors: Both registration and a licence to practise with the UK GMC. An up-to-date FSRH Letter of competence in Intrauterine Techniques (LoC IUT) and/or Sub dermal implants (LoC SDI) as appropriate. For nurses: Nursing part of the UK NMC register (excluding RN Level 2).
