Bristol, North Somerset & South Gloucestershire (BNSSG) Integrated Care Board (ICB) seeks to commission Discharge to Assess P2 & P3 Beds for patients in BNSSG. The procurement is split into lots according to bed type and the requirements of the region and to enable the spot purchase of P3 Bed requirements during the contract. BNSSG ICB has implemented a Discharge to Assess model following hospital admission, in collaboration with ICS partners. The overall model includes four pathways: Pathway 0 - 'simple' discharge to usual place of residence with no or low levels of ongoing support. Pathway 1 - discharge to usual place of residence with short term package of care to support rehabilitation and reablement (usually 10days), possibly leading to long term care support package most probably at home. Pathway 2 - intensive, bed-based rehabilitation (usually 21 days), requiring a community hospital or similar nursing home setting, possibly leading to long term care support package most probably at home. Pathway 3 - intensive, bed-based rehabilitation (usually 28 days), requiring a community hospital or similar nursing home setting, expected to lead to long term care package most probably in a nursing or residential care home. The vision of the model is to maximise independence, de-escalate care as soon as possible to prevent de-compensation and over-time this is expected to increase the number of people managing independently or with care support at home and reduce the numbers of people relying on bed-based care in the short term and long term. The Contract term will be 18 months initially, with the option to extend by any further period or periods up to a maximum of 12 months. The Contract is planned to commence on 1 October 2023. The total value of this procurement including extensions is £22,007,017.
