Caerphilly County Borough Council Social Services, Adult Services has the requirement for the Provision of Extra Care. Extra Care housing service enables people to retain control over their own lives, maximising opportunities for independence and social inclusion, while receiving the support they need in a safe environment within a community setting. People living in an extra care facility will expect to receive a level of support which is higher, or more flexible than would be provided in their own homes, but lower than would be provided in a residential or nursing care home. Full details can be found in the invitation to tender (ITT) documents Lot 1: The purpose of this service is to enable people aged 50 years plus who need housing, care, and support to maintain their independence, minimising the necessity for residential/nursing care. The support service will be provided for people at the following extra care housing facilities:- Cefn Glas, Maes Y Coed, Blackwood, NP12 1HD. Plas Hyfryd, Ffordd Y Maes, Caerphilly, CF83 2AU Full details can be found in the ITT.
